Notes to applicant

(A) Download the attached membership application form (2025).

(B) Submit your (1) completed and signed membership application form together with (2) a recent official photo (high resolution with not less than 3 MB) and (3) this slip and (4) payment (or proof) to the CityU Eminence Society Secretariat :-

(i)  via email (aas.cityues.sec@my.cityu.edu.hk) copies of items (1) & (3) only; AND

(ii)  at 12/F Elite Centre, 22 Hung To Road, Kwun Tong, Kowloon, Hong Kong all the above items (1) to (4).
