Membership Application Form

Please submit the following items to the CityUHK Eminence Society Secretariat:

  1. A completed and signed membership application form;
  2. A recent official high-resolution photo (not less than 3 MB);
  3. Proof of payment.

Submit these items by both methods below:

  1. Email scanned copies of items 1–3 to aas.cityues.sec@my.cityu.edu.hk; and
  2. Mail the original physical copies of items 1–3 to 
    Room 1-006, 1/F, CityU International Centre, 22 Cornwall Street, Kowloon Tong, Hong Kong (Attn: Miss Edith Ma)
